The Unfulfilled Wish in Honkai Star Rail is a unique achievement that can only be acquired after following a specific scenario. The achievement is part of the Eager for Battle category and will require players to unlock the playable character Blade. This means players who are yet to unlock the character will not be able to acquire the achievement titled Unfulfilled Wish in Honkai Star Rail. Needless to say, players can wait for a character rerun in the future if they wish to acquire the character in order to acquire this achievement in the game. How to unlock Unfulfilled Wish Achievement in Honkai Star Rail?
To unlock the Unfulfilled Wish achievement in Honkai Star Rail, players need to use Blade and have him restore his own HP from 5% to 30% and above in one single action two times. Healing from teammates does not count towards this achievement.
Here is a guide on how to easily acquire the Unfulfilled Wish achievement in Honkai Star Rail;
Select one boss fight in the Echo of War section of the Survival Index. Any of the available bosses will do.
Bring Blade with at least one Preservation-type character like Gepard and Trailblazer (Fire). The rest of the units are up to you.

Try to drop Blade’s HP to below 5% before he heals himself using his talent or ultimate ability.
When he is at very low HP, simply use Blade's ultimate to heal him back to 50%.
Repeat this process twice before killing the boss.

Players can bring a healer with them to keep the other characters alive. However, make sure to not heal Blade until you have acquired the achievement.
It is advisable to calculate how much is 5% of your Blade’s HP before proceeding. This will allow you to figure out when you need to cast your ultimate to heal him before dropping it back to 5%.

You do not need to defeat the boss in order to acquire the achievement. If you do not wish to clear the stage, simply retreat and claim your rewards after unlocking the Unfulfilled Wish Achievement in Honkai Star Rail.
