A Blizzard developer has clarified how players can secure Diablo 4's rarest unique items and it turns out that there are six Uniques in this category.
Those six extra-super-rare Uniques can drop from level 85+ enemies, and can come from anywhere you get a regular Unique.
The issue is that even the most devoted Diablo gamers are beginning to question whether these six things are simply too rare. Only a tiny fraction of the millions of players have even come across these things.

For the first week or two of Diablo 4, fans began to wonder if certain things supposedly in the game were actually included. These were iconic Diablo treasures like The Grandfather and Harlequin Crest, but only some people in the entire game had found them. When a couple of level 100 players obtained the Harlequin Crest, questions arose. When are these things expected to be released? How rare were they? Were there more? Blizzard has now stepped in to answer those questions, and it turns out that there are six Uniques in this category.

The game’s Lead Class Designer, Adam Jackson, came to Twitter to give out some details regarding the six items. Read below to find out about the 6 rarest items in Diablo 4.

Blizzard Confirms Six Rarest Items in Diablo 4

Diablo 4's Lead Class Designer Adam Jackson shared all six items on Twitter in an attempt to clarify where fans can earn these game changers and to provide a glimpse of which rare Uniques are currently available in the game.

The six rarest items all roll at 820 power, which explains why they’re so rare as very few items can even roll that high to begin with. They can only be obtained from enemies with a level of 85 or higher in the game. Because only the opponents must be level 85, you can theoretically obtain them from mid-level Nightmare dungeons. Here is a list of the six items revealed to be the rarest in Diablo 4

  • Doombringer (One handed sword) – Lucky Hit: Up to a (15-25%) chance to deal 5,822 Shadow damage to surrounding enemies and reduce their damage by 20% for 5 seconds.

  • The Grandfather (Two-handed sword) – Increase Critical Strike Damage by (60-100%)

  • Ring of Starless Skies (Ring) – Each consecutive Core skill cast reduces the Resource cost of your net Core skill by (8-12%) up to a maximum of 40%

  • Andariel’s Visage (Helm) – Lucky Hit: Up to a (15-20%) chance to trigger a poison nova that applies 10,077 Poisoning damage over 5 seconds to enemies in the are (also 1.3-2% life steal).

  • Harlequin Crest (Helm) – Gain (10-20)% Damage Reduction and +4 to all Skills

  • Melted Heart of Selig (Amulet) – Gain 30% Maximum Resource and when you take damage, drain (8-3) Resource for every 1% of Life you would have lost instead.

The issue is that even the most devoted Diablo gamers are beginning to question whether these six things are simply too rare. Only a tiny fraction of the millions of players have even come across these things. Simply put, it is impossible to "farm" for them in the conventional sense because you could probably level up ten characters to 100 without ever seeing them again. It has a "lightning-struck" possibility.
